The Wellbeing Team accepts referrals for patients (18+) registered at one of the following GP practices: Ansdell Medical Centre, Fernbank Surgery, Holland House Surgery, The Old Links Surgery, Parcliffe Medical Centre, or Poplar House Surgery.

Referrals for wellbeing interventions from the Associate Psychological Practitioner (APP) are open to patients of any age.

The PCN Wellbeing Team does not provide counselling or psychotherapy and is not for the treatment of severe or complex mental health needs. We recommend self-referral to MindsMatter or accessing the resources here
