Lytham St Anne’s Primary Care Network run a series of groups designed to help and support our GP practices.

Wellbeing Team

The Wellbeing Team consists of five Social Prescribing Link Workers, two Associate Psychological Practitioners, a Mental Health Practitioner, and a Macmillan Community Support Worker. Supported by one of the PCN Care Coordinators, the team work closely together to triage referrals and assess patients, to provide the best person-centred care for their needs.

The team accepts referrals from staff at our 5 GP practices, other professionals, and self-referrals from individuals themselves. Patients must be 18+ and registered at Ansdell Medical Centre, Fernbank Surgery, Holland House Surgery, Parcliffe Medical Centre, or Poplar House Surgery. Meet our wellbeing team.

Social Prescribing link workers

Social prescribing is a way of connecting patients (18+) to services and social groups in the local area in order to improve their overall health and wellbeing. Social prescribing link workers can spend time with a patient, finding out what really matters to them, and putting them in touch with the people and activities that might help them to feel better. Our link workers understand the social, economic, and environmental factors that affect our health, and have a good knowledge of supportive local resources in Lytham St Annes and the wider area. They work holistically to help patients take control of their own health and wellbeing.

The groups and activities that link workers connect patients with include friendship groups, walking groups, choirs, gardening, and volunteering opportunities. Link workers can support patients to the first session of a group if they feel nervous about going alone. Link workers can also support patients to navigate the services and organisations available for issues such as debt, housing, domestic violence, and employment.

People that might be helped by social prescribing include people who are lonely or isolated, unpaid carers, and anyone whose health is affected by non-medical issues.

 

Associate Psychological Practitioners

The Associate Psychological Practitioners (APPs) offer wellbeing interventions to patients who are struggling with mild mental health concerns such as low mood, low level anxiety, sleep problems. The APPs can provide advice and support with mindfulness, stress management, and learning coping skills.

Wellbeing Team Groups

Stronger Together

Stronger Together is a drop-in peer support friendship group for anyone who is struggling with bereavement or loss. It is facilitated by the PCN Wellbeing Team but is open to anyone regardless of whether they are registered at one of our practices.

The group meets every Friday afternoon at St John's Court in Lytham between 1pm to 2:30pm.

The group is friendly, informal, and give people the chance to meet others who may be in the same boat. Anyone who is interested is welcome to drop in during the group time, with no need to book. Alternatively, the team are happy to meet someone outside first if they are feeling a little nervous about coming in alone.

Men's Group

Men's Group is a drop-in "coffee and chat" friendship group for men. It is facilitated by the PCN Wellbeing Team but is open to anyone regardless of whether they are registered at one of our practices.

The group meets every Thursday afternoon at Liggard Court between 2pm to 4pm.

The group provides a friendly, informal space to meet other men. Once a month there is a quiz and hot pie. Anyone who is interested is welcome to drop in during the group time, with no need to book. Alternatively, the team are happy to meet someone outside first if they are feeling a little nervous about coming in alone.

Let's Talk Menopause

Our Let's Talk Menopause evenings are monthly, themed, Q&A sessions led by the wellbeing team with local clinicians.

 

The Q&A sessions are friendly an informal. Local NHS clinicians or other experts are on hand to answer questions, and there is a chance to chat to others about your experiences of menopause and perimenopause. The sessions are held montly on the third Tuesday of the month, but we have breaks over the summer and Christmas.
